Amending Enduring Power of Attorney and Lasting Power of Attorney Forms
On 5th May, the Office of the Public Guardian required all postal applications to be sent to the Office of the Public Guardian, PO Box 15118, Birmingham, B16 6QX instead of its London office.
The forms listed above will be amended to refer to this new postal address at their next reprint as amendment slips will accompany all orders placed for LPA (PA), LPA(PW)
and LPA 002.
Practitioners should note that all the existing EPA and LPA forms mentioned above can continue to be used indefinitely but should be posted to the new Birmingham address.
EXISTING FORMS CAN CONTINUE TO BE USED. AMENDED FORMS WILL BE MADE AVAILABLE AT THEIR NEXT REPRINT. PRACTITIONERS SHOULD SEND COMPLETED APPLICATIONS TO THE OPG’S NEW BIRMINGHAM ADDRESS.
