New and Amended Children Act and Family Law Act Forms

The above forms have been amended by various statutory instruments to reflect certain changes which the Human Fertilisation and Embryology Act 2008 will introduce on 1st September.C(PRA)3 is a new form to record an agreement where parental responsibility isacquired by a second female parent. 
 
CURRENT EDITIONS OF C(PRA1), C(PRA2), C13A, C19, FL401 AND FL401A SHOULD NOT BE USED AFTER 31ST AUGUST. AMENDED VERSIONS OF THESE SIX FORMS AND NEW C(PRA3) FORM WILL BE AVAILABLE TOWARDS THE END OF AUGUST.
