Filling-in HA21 & HA21A

To Insert the name and address of the tenant on whom the notice is being served. If there is more than one, give the name and address of each tenant.

From Insert the name and address of the landlord by whom the notice is being served. If there is more than one, it is not necessary for the notice to be given by each, but see section 2, para. 7.

Address of dwelling Insert the address or identifying description of the property as it is stated in the agreement form which has been signed.

"after" In the case of HA 21 this date must be at least two months after the date when the notice is given. In the case of HA 21A if must be (i) at least two months after the date when the notice if given and (ii) the end of a period of the tenancy. If you send the notice by post rather than delivering it by hand to the property, you are well advised to send it by recorded delivery and to allow 48 hours for delivery.

Dated Insert the date when the notice is signed.

Landlord The landlord or the landlord's agent should sign the notice in the space immediately above the word "landlord".

Landlord's agent If the notice is being served by the landlord's agent, the agent's name and address should be inserted at this point.
